A Look Up Close At Jaundice

In recent weeks BU’s resident medical expert Doc GP has submitted presentations to educate the family about cancer. Some may feel it is because the Prime Minister is rumoured to have the Big C explains our focus. BU says so what? Now is as good a time as any to educate on the issue of the Big C, it is generally accepted that when a prominent or high profile person contracts any disease, engage in or suffers physical abuse or any malady, the public’s curiosity is heightened.
Against this background the Doc has submitted a presentation on the subject matter ‘JAUNDICE’!

I thought you were a fast bowler, but now ya like ya spinning. LOL
Anyway, some American medical student who had the opportunity to attend UWI medical school for one year, wrote on a Medical Student forum yesterday thus
“At UWI if they put you in a room with a patient and a stetoscope you can get the diagnosis. All the fancy technology will do is confirm the diagnosis.”
HE IS OFF COURSE ABSOLUTELY CORRECT!
It is with that background that non doctors but rapidly improving UWI-esque medical students like Crusoe and Hants can use information on the web and BU classroom teachings to accurately come up with the diagnosis of a person in a photo who has yellow eyes and yellow skin.
If these non doctors but rapidly improving medical students can do this, one wonders why a certain moron on another thread was criticizing BU for posting articles on “certain cancers” and on staging grading and metastasis.
On any given day on BU you can be in Bible School, Law School, Medical School or any other school.
I take this liberty to say that if you dont want to be educated at such a high level, you can blog elsewhere at other places where you cant get good milk far less food!

The diagnosis of the man in the photo is thus
JAUNDICE (querry cause)
In other words he got jaundice and doctor have to find the cause.
We can start by ruling out the congenital causes on the list and the causes due to pregnancy. Right?
If you read the notes you can come up with the most likely diagnosis.
I dont think we have leptospirosis on the list. That can kill ya fast so get the Ministry of Health to get rid of the rats I hear that tekkin over Bim.
Alcohol can make you jaundiced too you know. By that time you might as well have cancer cause the result is the same!
So watch the rats and the lickers fellas!
Alcohol is a good bet. Doing the cocktail circuits would have increased big time in the last 2 years. The PM we understand is a Vodka man.

Here is a brief ditty on your question. Hope it helps
• The barrier function of the gut has been shown to be defective in obstructive jaundice.
• Surgical intervention in jaundiced patients is associated with a high incidence of post-operative complications, multiple organ failure and a high post-operative mortality.
• It is thought that these complications result from failure of the gut barrier, allowing bacteria and endotoxins normally contained within the gut to cross into the circulation, activating a systemic inflammatory response which causes damage to multiple organs.
• Recent research group confirms that permeability of the gut barrier is increased in jaundiced patients and that this returns to normal when bile is returned to the gut by internal biliary drainage. Evidence of bacterial translocation and immune dysfunction in obstructive jaundice has also been demonstrated
There is accumulating evidence that multiple organ failure is not always the result of an established septic focus i.e infection
Increasing attention has centred on the gut as a reservoir of bacteria (and bacterial endotoxins) that can traverse the intestinal mucosal barrier (a process called bacterial translocation ) and initiate the septic state.
i.e normally the gut protects us from bacteria that we take in with our food and the toxins that some of these bacteria produce. The barrier function of the gut has been shown to be defective in obstructive jaundice because bile is an irritant.
So when the free flow of bile from the gall bladder into the duodenum is prevented, bile which is an irritant backs up in the pancreatic ducts.
These ducts were not designed to handle bile fluids and therefore become severely inflamed thereby promoting infection. This makes matters worse, and can kill the patient before the malignancy does. One of the points made in the ppt on metastasis.
Oh by the way that ppt is a complilation of efforts from some of my med students five years ago
Thanks GP. These medical matters are posted on the WWW and can be refuted by anyone. In 3 years BU has not had reason to doubt the efficacy of your medical contributions. BU urges you to continue you to facilitate the learning you have started on BU and avoid the tangential forages which you seem to be easily lured.

David this is how you do the cocktail circuits.
Before you even start make up your mind that you will NEVER put more than 2 alcoholic drinks in your body in any 24 hour period (one drink if you are a woman) At the cocktail party do the one drink early, then do some plain ice water (when it is in your glass who but you can tell that it ain’t vodka), listen a lot, talk very little, smile a lot, greet everybody. Just before ya leave do the other drink. Or if ya have 2 or more functions in an evening, one drink per function, then nothing but ice water, smile, listen (in other words make other people feel good)
Remind yaself all the time that the world will NEVER run out of alcohol. We born and find it and we will ALL die and leave it. So never feel it is your job to drink it all up, because you can’t. But ya might kill yaself in the effort.
A friend of mine who has done the cocktail circuit for nearly 40 years gave me this advice. The friend still in great shape (ALL parts of said friend)
If everyone followed your good advice, cocktail parties would consist of lots of smiling, silent people drinking water! Better to just stay home and get to bed early.
That way you wont get jaundice secondary to alcoholic hepatitis or cirrhosis or chronic pancreatitis or cancer of the pancreas.
